Our Community Health Worker Program is a critical component of the Outreach and Education elements of CHOICE’s core mission. Having workers from within the communities we serve who have experienced the same health events as our patients put us in a unique position to build rapport, comfort, and trust. Those elements lead to excellent and long-lasting treatment compliance and healthier lifestyle choices thanks to the health education program.
